Case 4 Flashcards

1
Q

Patient Ch., a 28-year-old teacher, called a doctor due to poor health and the inability to continue working at school. The patient had been ill for 3 days. The disease started with rhinitis and cough, and soon the body temperature rose to 38 °C. The patient complained of headache, weakness, sore throat, pain in the eyes, and tear secretion. Objective examination findings: nasal congestion, sneezing, wet cough, scarce mucous and bloody nasal discharge, skin irritation under the nose and above the upper lip were noted. The mucous membrane of the pharynx was moderately hyperemic and loose, the tonsils were enlarged, mucoid depositions in the lacunae. Hyperemia of the conjunctiva, mostly on the left, was noted. Enlarged submandibular and anterior cervical lymph nodes were palpable. There was no pathology in the lungs and heart. The heart rate was 80 beats per minute. Blood pressure was 110 / 70 mm Hg. The abdomen was painless, the edge of the liver was at the costal margin. There were no meningeal symptoms. Physiological functions were normal. Task 1. What is the most likely diagnosis?

A

Q
  1. What are the features of the pathogenesis of this disease?
A

Features of the pathogenesis: Adenovirus infection is caused by the adenovirus, which is a common cause of respiratory and gastrointestinal infections. The virus is transmitted through respiratory droplets, close contact, or fomites. After the virus enters the body, it infects the respiratory or gastrointestinal epithelium, leading to inflammation and tissue damage. The immune response to the virus can also cause symptoms such as fever, cough, and sore throat.

Q
  1. Make an examination plan for the patient taking into account the spectrum of differentiated diseases
A

Examination plan: The examination plan for a patient with suspected adenovirus infection should include a thorough medical history, physical examination, and laboratory testing. Laboratory testing may include PCR or viral culture to confirm the presence of adenovirus. Differential diagnosis should also be considered, including other respiratory viruses such as influenza, parainfluenza, respiratory syncytial virus (RSV), and coronavirus.

Q
  1. List possible complications of this infection.
A

Possible complications: Complications of adenovirus infection can include pneumonia, bronchitis, sinusitis, and otitis media. In severe cases, adenovirus infection can also cause hepatitis, myocarditis, and encephalitis. Adenovirus infection can also exacerbate underlying medical conditions, such as asthma or chronic obstructive pulmonary disease (COPD). In immunocompromised individuals, such as those with HIV or undergoing chemotherapy, adenovirus infection can cause severe and potentially life-threatening complications.
