What are the stages of Case Management?
Engagement, Assessment, Planning, Accessing Resources, Coordination, Disengagement
Define engagement
the process of building an effective working relationship (reaching out) -
Methods include:
Interviewing techniques
Eliciting info about probs
Handling neg feelings
buil;ding trust (2 way street-dialogue goes both ways)
Clarifying role expectations
Define assessment
-figuring out what the consumer needs *traditional problems)
WHAT RESOURCES ARE AVAILABLE
FORMAL - dhhs dEPT OF lABOR ETC AND I NFORMAL - FAMILY, FRIENDS, COMMUNITY SUPPER
BARRIERS - EXTERNAL, INTERNAL, INHERENT INCAPACITIES
EXTERNAL - resource doesnt exist - ie rural areas
or secondary resource needed to access first resource which is unavailable
INHERENT INCAPACITIES - factors outside ofapersons control which reduce or eliminate the persons ability to communicate effectively with the helpers ie MR MI Stroke alcoholism or drug abuse TBI
INTERNAL - beliefs attitudes or values that interefere with the consumers way of seeking help ie
Pessimism , Criticism, Fatalism, Cynicism
3 Planning -
taking info from assessment and develop a PLAN OF ACTION
4 ACCESSING RESOURCES
implement the service plan by using interventions to overcome barriers and connect client to needed resources (AT CLIENTS PACE)
5 COORDINATION
1 insure help remains available (support)
2 monitor plan of effectiveness (follow-up)
3 task implementation sequence (prioritize)
4. contracting to enhance motivation (Clarity)
6 DISENGAGEMENT
resolution of problems - ability to get and use help effecvtively - initiates helping ocnnections witnout case manager assisstance
