Adaptive to multiple practice settings in which case managers function, and the setting in which patient receive their care and services.
What are the 3 primary goals of Case Management?
- Improving clinical, functional , emotional and psychosocial health of patients as a whole, impacting patients with the complex health needs served at a single point of contact.
- Links patients to providers and resources based on their cultural beliefs, values and identified needs.
- Can improve outcomes when care is appropriately managed, efficiently provided and effectively executed.
What is the first duty of the case manager to the patient?
