Color/Clarity: straw, yellow, amber, red, tea colored/ clear, sediment, clots

Teach the patient about what you are doing and assess patient’s ability to assist with the procedure
Position
patient in recumbent position
Verbalize
Pericare. “I would then put on clean gloves and clean the perineal area from front to back. Then i would rinse and dry. Remove my gloves and wash my hands”
Open
Sterile kit
Apply sterile drape
Apply drape and done sterile gloves
Place
sterile tray between thighs
Open suplies
Ensure clamp on bag is closed Fluff cotton balls on tray and pour antiseptic solution over them Set specimen container aside Open lubrication, pour on tray Insert sterile water syringe
Lubricate
1 to 2 inches of catheter tip
With non-dominant hand
spread the labia. DO NOT LET GO.
Use dominant hand
to clean both labial folds and meatus
Dominant hand
position catheter and setup within easy reach on sterile feed
Inserting
hold 2-3 inches from tip.
do not force catheter
ask patient to breathe deeply and rotate catheter slightly if there is slight resistance
insert until urine drains, then advance 2-3 more inches
Hold
catheter securely at the meatus with non-dominant hand
inject sterile water
pull gently to feel resistance
remove syringe with plunger depressed so fluid remains in balloon
Verbalize
remove equipment and dispose of it according to facility policy
Wash and dry perineal area
Remove
gloves and perform hand hygiene
Secure catheter tubing to patients inner thigh
Leave slack in catheter for leg movement
