What part is the foundation of a word and gives the basic meaning of it?
Word root
What is used when writing the word root by itself?
Combining form
When is a combing vowel used to put together word parts?
Between 2 word roots even if 2nd root begins with a vowel, and between a word root and suffix if suffix doesn’t begin with a vowel.
What does a prefix do?
Usually gives information about the location of am organ, the number of parts, or the time(frequency).
What does the suffix do?
Adds meaning such as a condition , disease or procedure.
History and physical
Done by admitting physician about clients history, results of examination, initial diagnosis, and plan of treatment.
Physicians orders
Complete list of care, meds, tests, and treatments physician orders for client.
Nurse notes
Record of care throughout the day done by the nurse.
Physician progress notes
Daily record of clients condition, result of exam, summary of test results, further plan for care.
Consultation reports
Reports by specialists referred by doc.
Ancillary reports
Reports from various treatments such as respiratory therapy, rehabilitation, social services.
Diagnostic reports
Results of tests such as labs or medical imaging.
Informed consent
Document voluntarily signed that explains procedure, benefits, and risks of treatment or procedure.
Operative report
Report by surgeon on operation, post and preoperative diagnosis, details of procedure, and tolerance of patient.
Anesthesiologist report
Includes vital signs, fluids and meds given, and patients tolerance.
Pathologist report
Report by pathologist that studied tissue removed from patient.
Discharge summary
Outline of entire patients stay, like condition on admin, diagnosis, test results,treatments, final diagnosis, and follow up plans.
Acute care or general hospital
Services that diagnose and treat for a short time period. They provide obstetric and emergency care.
Specialty care hospital
Specific type of disease. Like psychiatric.
Nursing home long-term care facilities
Provide long term care for pts ending extra time to recover, or those that can no longer take care of themselves.
Ambulatory care centers, surgical centers, or outpatient clinic
Give service that don’t require over night stay.
Physicians offices
Give diagnostic testing or therapy
Health maintenance organization (HMO)
Provides wide range of services by a group of pops, specialists, in a prepaid system.
