Word statement so it’s legally advisable.
Use nonjudgmental statements.
Make sure both elements of the statement do not say the same thing.
Be sure that the cause and effect are correctly stated.
Word the dx specifically and precisely.
Use nursing terminology not med to describe the clients response and probable cause.
