Reports of special importance or of a nature similar to the following shall be preceded by an immediate telephone communication to the DC and BC on duty. These are as follows

A
  • Unusual occurrence at a fire or elsewhere
  • Loss of life or serious injuries to members or civilians at a fire or elsewhere
  • Accidents to apparatus, or the placing of a company or apparatus OOS
  • Structural or occupancy hazards imminently dangerous to life or property
  • Depletion in a unit of manual strength required for fire duty

Unless otherwise directed in department directives, orders, rules and regulations the following instructions shall be adhered to

  • Reports shall be forwarded in single copy
  • Reports shall be promptly forwarded thru official channels
  • Reports shall be addressed to Chief of Operations

Whenever a FD unit or member is involved in an incident or unusual occurrence with a member of another city agency, an Unusual Occurrence Report is to be prepared by the officer of the umpteen involved. What are the procedures

A
  • Report to contain all the particulars including names, ID numbers, time, location, etc
  • This report is to be FAXED to the Bureau of Operations by 0900 hours the following business day. On weekends/holidays the report is to be FAXED to the City Wide Command Chief. Proper and full endorsements are required
  • Administrative DC’s can authorize OT to those members required to prepare this report within the prescribed time frame. The officer receiving authorization shall make appropriate journal entries
