Glaucoma

A

Build up of pressure in the eye caused by blockage in the pathway in the front of the eye. Coded based on stage and type of angle closure.

63
VF
Visual field
64
WNL
Within normal limits
65
Evisceration of the eye
Contents of the eye are removed but the sclera shell and muscles remain so a prosthesis can be fitted and will have natural movement
66
Enucleation of the eye
The connections and the entire is removed en masse.
67
Exenteration of the eye
Surrounding skin, fat, muscle, and bone are removed. Most extreme type of surgery and reserved for serious malignancies
68
Lamellar
Refers to partial thickness of the cornea
69
Pterygium
Benign growth of the conjunctiva attached to the sclera and extends from the inner canthus to the border of the cornea with an apex that points to the pupil. May require surgery if blocking the field of vision.
70
Keratomileusis
Procedure to alter visual acuity. A partial thickness portion of the cornea is frozen, reshaped, repositioned and sutured back into place. Largely been replaced by photorefractive keratectomy and lasik.
71
PRK and lasik codes
66999. Include a letter and operative report for this misc code.
72
Keratophakia
Trephine is used to punch a measured circular hole in the cornea of the eye. Incision is made at the juncture of the cornea and sclera. Patients cornea is separated into two layers and donor cornea is inserted between the two layers correcting refractive errors. Usually cosmetic and not covered.
73
Epikeratoplasty
Trephine punches measured circular hole in the cornea of the eye. A lens made of the stroma and Bowman’s layer is shaped on a lathe. Lens is sutured in place correcting a preexisting refractive error.
74
Keratoprosthesis
New anterior chamber is created with a plastic optical implant replacing and severely damaged cornea.
75
Radical keratotomy
Patient’s cornea is measured and multiple, non-penetrating cuts are made in a bicycle spoke pattern on the cornea to reduce myopia. A variety of peripheral cornea tangential cuts may be made for astigmatic correction.
76
Cyclectomy
Removing part of the ciliary muscle along with the lesion
77
Intracapsular cataract extraction (ICCE)
Physician removes the cataract and capsule as one and inserts a lens prosthesis. Seldom performed.
78
Vitrectomy
Surgical removal of vitreous fluid.
79
Lagophthalmos
Inability to fully close the upper eyelid which can result in damage to the cornea by drying out its surface due to lack of moisture.
80
Ectropion
Turning outward of the margin of the lower eyelid.
81
Entropion
Inversion of the margin of the eyelid.
