chapter 6 data management Flashcards

1
Q

what is data source?

A
  • Locations where data is being generated and stored within an organization.
  • Includes both:

Q

what is system characterization?

A
  • inventory of all systems that contain data
  • A process of creating an inventory of all systems that contain data, including documenting where the data are stored, what type of data are created or stored, how they are managed, with what hardware and software they interact, and providing basic security measures for the systems.

Q

what is data vs information?

A
  • data
    o raw facts
    o EX: 40 degrees Celsius
  • information

Q

what is a disease index?

A

○ A listing specific codes such as international classification of diseases, tenth revision, clinical modification (ICD-10-CM) codes that link a disease or diagnosis to a patient. Common data in a disease index would include diagnosis codes, health record number, gender, age, race, attending physician, hospital service, patient outcomes, and dates of encounter. A disease index can be used to query a specific diagnosis to determine other attributes of patients with the disease.

Q

what is operation or procedure idex?

A

○ A listing of specific codes such as current procedural terminology (CPT) for procedures or operations performed within the healthcare organization. An operation or procedures code would include information similar to the disease index but would also include the specific code numbers as well as the operating physician.

Q

what is a physician index?

A

○ A listing of all physicians within a healthcare organization with all the diagnosis and procedure code linked to each provider within the index. The data collected in this index include physicians’ identification (code or name), health record number, diagnosis, operations, dates of service, patient gender, patient age, patient outcome from encounter. A provider index can be used to produce information regarding the work of the provider within a healthcare organization, which can be useful for certification and credentialing purposes.

Q

what are the two system of data mapping?

A

o Source data:
 Location from which the data originates, such as a database or a data set.

o Target data:
 Location from which the data is mapped or to where it is sent

Q

what is data warehouse?

A
  • The process of collecting the data from different data sources within the organization and storing it in a single database that can be used for decision making.

a database that makes it possible to access data that exist in multiple databases through a single query

Q

what is information governance?

A
  • focuses on principles and oversight to manage the information that is produced from different systems within an organization.
22
Q

what are information assets?

A
  • data, documents or other information resources that are valuable and managed by the organization.
23
Q

what is data intergrity?

A
  • concept includes the process of data governance, patient identification, authorization validation, amendments and record corrections, and audit validation
24
Q

what is a data stewardship?

A

o Creation of responsibility for data through principles and practice to ensure that the appropriate knowledge and use of the data from personal health information is being used appropriately

25
what is data steward?
o the people within the organization who are responsible for either a specific system or a specific set of data within the organization.
26
what is interoperability?
* the capability of two or more information systems and software applications to communicate and exchange information called
27
What is metadata?
* example is an audit trail * refers to managing and defining, that the metadata within the information system. Metadata refers to the data that characterizes other data such as creation date of data, date sent, data received, last accessed date and last modification date. It is important to clearly define what metadata will be collected and why it will be collected.
28
what is critical thinking?
* process of analyzing assessing, and reconstructing a situation to provide enhanced solutions and outcomes to be a problem
29
what are the two clinical forms committe?
* Forms Design * EHR Screen Design
30
what is clinical documentation integrity? (CDI)
* a process to conduct concurrent and retrospective reviews of medical information to review for conflicting, incomplete, or nonspecific provider documentation in order to verify clinical specific and documentation is appropriate and supports the medical codes assigned the process of revewing medical information to verify that documentation is clincal specific, is appropriate , and supports medical codes assigned.
31
what is query?
tool used in CDI
32
what are the 3 query tools?
* Computer assisted coding (CAC) o Software that can search and evaluate clinical documentation to provide information regarding potential areas for documentation integrity * Audits: o Help an organization determine where there are areas that are missing proper documentation * Quaries: o A communication tool for CDI staff to communicate with the providers to obtain clinical clarification comma provided documentation alert comma clarify documentation comma or ask additional questions regarding the documentation o Paper, electronic Quaries
33
what is accuracy ?
■ The extent to which the data are free of identifiable errors
34
what is accessibility?
proper safeguards must be established and employed to ensure the data are available when needed while implementing proper precautions and safeguards to protect the information.
35
what is comperehensiveness?
that all required data elements that should be collected throughout the health record are documented.
36
what is consistency?
patient data within the health record should be the same and should not contradict other data also in the health record.
37
what is currency?
the data within the health record a need to be current and up to date.
38
what is definition?
■ All data elements should be clearly defined to guarantee that all individuals using and collecting the data will understand the meaning of that data element.
39
what is granularity?
the data collected for patient care must be at the appropriate level of detail. documenting the results of a lab test with appropriate number of characters after the decimal point in the lab value
40
what is percision?
■ Data should be precise and collected in their exact form within the course of patient care
41
what is relevancy?
■ The extent to which healthcare- related data are useful for the purpose for which they were collected.
42
what is timeliness?
■ Concept of data quality that involves whether the data is up-to-date and available within a useful time frame; timeliness is determined by manner and context in which the data are being used.
43
what is structured data?
o Are data that can be read and interpreted by an information system o EX: a diagnosis code entered in the proper format into the information system such as am ICD-10-CM code format XXX.XXXX
44
what is unstructured data?
o Also known as free text are data entered into the information system with no format specified. o EX: narrative discharge summary that does not follow a specific format or use a template.
45
what is medical staff bylaws?
o Describe the manner in which providers will practice medicine within an organization that algins with the mission and values of the organization.
46
what is hospital by laws?
o Written documents that govern the staff members who create data within the record for additional support of patient care and reimbursement
47
what is data management?
combined practices of HIM, IT and HI that affect how data and documentation combine to create a single record
48
what is a database?
a collection of data organized in such a way that its contents can be easily accessed, managed, reported and updated
49
what is an index?
a report of list from a database that provides guidance or other references to that data in the database.
50
what is data mining ?
processing of extracting from a database or warehouse information stored in discrete, structured data format
