what is data source?
- Locations where data is being generated and stored within an organization.
- Includes both:
o Clinical data EX: test results and history
o administrative data elements EX: billing data and demographic
what is system characterization?
- inventory of all systems that contain data
- A process of creating an inventory of all systems that contain data, including documenting where the data are stored, what type of data are created or stored, how they are managed, with what hardware and software they interact, and providing basic security measures for the systems.
what is data vs information?
- data
o raw facts
o EX: 40 degrees Celsius - information
o processed, organized and given context to become meaningful
o the patients’ temp is 40 degrees
what is a data element ?
● A single or individual fact that represents the smallest unique subset of a larger database sometimes referred to as the raw facts and figures.
● EX: Age, gender, and blood pressure
what is data dictionary?
- A listing of all the data elements within a specific system that defines each individual data element, standard input of the data element, and specific data length.
what is data sets ?
a recommended list of data elements that have defined and uniform definitions that are relevant for a particular use or are specific to a type of healthcare industry.
UHDDS (data set)
○ Uniform hospital discharge data set
○ Created by NCHS
○ Collects uniform data elements from the health records of every hospital inpatient.
ex: date of birth, sex, race, admission date
UACDS (data set)
○ Uniform ambulatory care data set
a standardized data set for the ambulatory setting
○ Outpatient setting
○ Collects data specific to ambulatory care settings with an intent to improve data comparison across different settings of healthcare.
ex:-Provider identification, address, type of practice

what are the two databases?
● Relational
○ Data stored in tables with rows and columns
● Object-oriented (OODB)
○ Data is represented and stored in the form of objects
images, audio, videos
○ Can store more complex data
what is master patient index (MPI)?
○ A guide to locating specific demographic information about a patient such as the patient’s name, health record number, date of birth, gender and dates of service.
what is a disease index?
○ A listing specific codes such as international classification of diseases, tenth revision, clinical modification (ICD-10-CM) codes that link a disease or diagnosis to a patient. Common data in a disease index would include diagnosis codes, health record number, gender, age, race, attending physician, hospital service, patient outcomes, and dates of encounter. A disease index can be used to query a specific diagnosis to determine other attributes of patients with the disease.
what is operation or procedure idex?
○ A listing of specific codes such as current procedural terminology (CPT) for procedures or operations performed within the healthcare organization. An operation or procedures code would include information similar to the disease index but would also include the specific code numbers as well as the operating physician.
what is a physician index?
○ A listing of all physicians within a healthcare organization with all the diagnosis and procedure code linked to each provider within the index. The data collected in this index include physicians’ identification (code or name), health record number, diagnosis, operations, dates of service, patient gender, patient age, patient outcome from encounter. A provider index can be used to produce information regarding the work of the provider within a healthcare organization, which can be useful for certification and credentialing purposes.
what is data mapping ?
- A process that allows for connections between two systems
EX: ICD-9 & ICD-10
what are the two system of data mapping?
o Source data:
Location from which the data originates, such as a database or a data set.
o Target data:
Location from which the data is mapped or to where it is sent
what is data warehouse?
- The process of collecting the data from different data sources within the organization and storing it in a single database that can be used for decision making.
a database that makes it possible to access data that exist in multiple databases through a single query
what is information governance?
- focuses on principles and oversight to manage the information that is produced from different systems within an organization.
what are information assets?
- data, documents or other information resources that are valuable and managed by the organization.
what is data intergrity?
- concept includes the process of data governance, patient identification, authorization validation, amendments and record corrections, and audit validation
what is a data stewardship?
o Creation of responsibility for data through principles and practice to ensure that the appropriate knowledge and use of the data from personal health information is being used appropriately
