A

Medulla - Phaechromocytoma - Excess adrenaline
Fasciculata - Cushings - Excess cortisol
Glomerulosa - Conns - Excess aldosterone

All present with hypertension and low K and high Na (less so for phaeo)

A

It is basically excess cortisol

Can be due to:

  • Adrenal tumour
  • Pituitary tumour producing ACTH
  • Ectopic tumour producing ACTH
  • Oral steroids

How do you test for Cushings

A

9 am cortisol
12 midnight cortisol
Dexa suppression test. - You are supposed to switch off ACTH and cortisol

How do you do a dexa test?

A

Measure ACTH and cortisol at the start
Give 0.5mg dexa every 6 to 48 hours
Cortisol should fall to <50nM

What is the difference between cushings syndrome and disease?

A

Syndrome - can be multiple causes

Disease - Pituitary DEPENDANT

A

Hypertension, bruising, low K, high Na

A

Obese people can present with high cortisol

How do you determine the cause of Cushings syndrome after dexa test

A

Pituitary sampling
