Cholecystectomy Flashcards
(9 cards)
… (ICG) fluorescence imaging to enhance biliary anatomy visualization during gallbladder removal.
Indocyanine Green
(The liver absorbs ICG and excretes it into bile, making the biliary anatomy glow under near-infrared (NIR) imaging)
Why is ICG flourescence imaging used?
Helps to identify the cystic duct, common bile duct and hepatic ducts
reduces risk of bile duct injury
(especially helpful if there is lots of scaring, obesity, or prior surgery)
what are the steps to RAL cholecystectomy?
- reverse trendelenburg w/left tilt
- port placement
- ID critical structures
- Critical View of Safety (CVS)
- Cystic Duct & Artery Ligation
- GB Dissection from Liver Bed
- Specimen removal (umbilical port)
- Irrigate, hemostasis, close
Critical View of Safety (CVS)
Identify & clear the hepatocystic triangle (cystic duct, cystic artery, liver edge)
Persistent RUQ pain after cholecystectomy + jaundice → …(2 most-likely complications)
- bile duct injury
- retained stone
What are the 5 MC complications of RAL Cholecystectomy?
- Bile duct injury
- Hematoma
- Bowel or liver injury
- Port-site Hernia
- Post-cholecystectomy syndrome
RUQ pain + fever + jaundice (charcot’s triad) → … (most likely diagnosis)
Suspect ascending cholangitis
ascending cholangitis tx
IV antibiotics + ERCP
If bile duct injury is suspected postoperatively (cholecystectomy) → Order … or … to assess bile leak.
- MRCP
- HIDA scan
