(38 cards)
Murphy’s sign
Inspiratory arrest with deep palpation of upper right quadrant
Think: cholecystitis
Pain that waxes and wains…. Think:
Kidney stone
Costovertebral angle tenderness….. Think:
Pyelonephritis
“Cupping” think:
Glaucoma
Normal Rinne test
Air conduction> bone conduction
Weber test
Sound heard equally in ears
Hot potato voice think:
Epiglottitis or peritonsillar abscess
Trismus
Painful to open jaw.
Peritonsillar abscess
Herberdens nodes
Distal interphalangeal
Think osteoarthritis
Bouchards nodes
Proximal interphalangeal
Osteoarthritis
Transient, symmetrical, joint stiffness that gets worse with activity
Osteoarthritis
Early morning symmetrical stiffness better as day progresses
Rheumatoid arthritis.
Synovial thickening of the metacarpophalangeal joint. Hand swelling
Rheumatoid arthritis
Tinel sign
Carpal tunnel syndrome.
Tapping over nerve on the wrist produces pain and parenthesis proximal or distal to the site of compression.
Phalen test
Carpal tunnel syndrome.
Wrist flexion for 30-60 seconds reproduces pain or parenthesia because of nerve compression in the carpal tunnel.
(elbows out, fingers down, wrists together)
Osteoporosis rx:
Daily calcium 1500mg/day
Daily vitamin D 1000 iU/day
Weight bearing exercise.
+ Neer test:
“Passive painful arc maneuver”
Shoulder impingement syndrome.
Raise arm (flex glenohumeral joint) while simultaneously preventing shoulder struggle. Will produce pain
Hawkins Kennedy test
Tests shoulder impingement syndrome.
Stabilize patients shoulder with one hand and with the pt elbow flexed at 90 degrees internally rotate the shoulder with the other hand. + is pain elicited by internal rotation.
Painful arc sign
Rotator cuff pathology.
Pain with raising arm above 90 degrees
+ drop arm test
Rotator cuff syndrome.
Anterior drawer sign
ACL injury.
Pt supine. Leg 90 degrees. Sit on foot. Grab proximal tibia both hands and pull anteriorly. Laxity= +
Lachman’s test
ACL injury if + laxity.
Place knee 30 degrees flex. Stabilize femur with one hand and pull proximal tibia anterior with other hand. Look for lack of endpoint.
Pivot shift test / vagus stress test
ACL injury.
Extend leg straight out. Hold leg and internally rotate tibia while putting vagus stress on knee with other hand. If there is reduction or a clunk then it’s +
Appley’s grind test
Meniscus tear
Lay on stomach. Flex knee 90 degrees. Place finger over media or lateral joint at knee and push down on tibia. Rotate knee in and out in rolling motion. If you hear a click or pop it is +
