Mechanical lift for dependent resident
Max A
Maximum assist, caregiver is expecting to physically perform 75% or more of the activity while the patient is expected to perform 25% or less
Mod A
Moderate assist, both the caregiver and the patient are expected to physically perform 50% of the activity
Min. A
Minimum assist, caregiver is expecting to physically perform 25% or less while the patient is expected to physically perform 75% or more
CGA
Contact assist, anytime a caregiver feels that guarding during an activity requires contact
SBA
Stand by assist, usually requires verbal cueing of caregiver for time of day or what activity is required for safe technique
I
Independent, no cueing, supervision or physical assistance of caregiver is required for any activity
T:
Transfers
How the patient moves to and from the chair or wheelchair
How the patient moves to and from a standing position
A:
Ambulated
How the patient moves from place to place; do they walk or use a wheelchair ect.
M:
Mobility
How the patient moves to and from lying position
How the patient moves/scoots up in bed
How the patient moves/turns side to side
How the patient moves positions their body while in bed, recliner or type of furniture the patient sleeps in
How the patient gets into bed

Precautions
Areas of concern for that patient, including fluid restrictions, skin issues,alarms,fall risk,and any other precautions that care givers must be aware of
