head, heart, vessels, drugs
Give DDx diagnoses for causes of collapse in head?
Hypoxia
hypoglycaemia
epilepsy
dysfunction of brainstem - stroke, TIA
Give DDx diagnoses for collapse with the heart?
Heart problems i.e. IHD emboli aortic obstruction rythm disorders tachyarrhtymias
vessel causes of collapse
vasovagal ENT situational sensitive carotid sinus ectopic pregnancy low vascular tone subclavian steal
What is the San Francisco syncope rule?
If someone has true syncope and any of the following features, admit - high 7 day mortality: CHESS C - history of cardiac failure H - hemotocrit < 39% E - abnormal ECG S - shortness of breath S - systolic <90
What is the OESIL risk score?
Works out risk of cardiac death
What OESIL risk score increases the risk of cardiac death?
> 2
What factors influence the OESIL risk score?
age >65 years
history of CVS disease
syncope w/o prodomes
abnormal ECG
What’s the neurological assessment after collapse?
cranial nerves, peripheral nerves, cerebellar, gait, AMTS, fundi, NIHSS (risk stratification if need to thrombolysed), GCS, pupils, lateralising signs, cap glucose
