What is a Qualifying Vehicle ?
- A commercial vehicle that has a Gross Weight Rating or Gross Combined weight rating of MORE THAN 10,000 LBS
- Any vehicle displaying a hazardous material placard
- A bus designed for nine or more passengers including the driver
Where do you record the VIN?
PAR on FORMS will be recorded in the stand alone caption of the PAR
PAR handwritten VIN will be written in the “Accident Description/Officer’s Notes” section of the PAR
PAR Property Only
-Position the RMP behind Vehicles involved so that traffic will NOT be impeded
- Provide EACH motorist with
- Report of Motor Vehicle Accident
- Collision Info Exchange Form
- What you should know about vehicle collisions cars
- Inform the motorist to prepare and forward the Report of Motor Vehicle Accident to the NYS DMV within 10 days if property damage is over 1,000. (Failure to do so may result in license or registration suspended)
- Instruct the motorist to exchange information and provide radio code 10-99T3 Referred to DMV and resume patrol/assignment
What gets Recorded on a PAR (Not property only)?
- Any person injured, incapacitated, trapped, or deceased.
- Any vehicle towed from the scene
- Motorist unable/unwilling to exchange information
- Disputes
- Damage to property other than vehicles involved
- Qualifying Vehicle
- Suspected Criminality
- City involved
- Deer struck or domestic animal
SPI + STV= Preliminary Breath Test
-Will be administered to all operators involved if qualified to do so (unless operator is removed from the scene for immediate medical attention)
Results or refusal of PBT go in Details of the PAR.
When do you prepare a Truck and Bus Supplement Police Accident Report ?
Need a Qualifying Vehicle AND a condition of severity
Condition of Severities
- Any vehicle is towed/transported (not for a Flat Tire)
- At least one person sustained Fatal Injuries (Death)
- At least one person was removed for medical treatment
EACH Qualifying vehicle gets the MV104S form and will be attached to the PAR
How do you obtain a PAR?
- Electronically or by Mail through NYSDMV
- At the precinct of occurrence within 30 days with a valid form of Identification
- Through mail to precinct of occurrence within 30days with a self addressed STAMPED envelope and has to have copy of identification
- Through Departments Public internet website
Do you always request the Patrol Supervisor for Bicycle Collisions?
Only if it involves:
- Death
- Seriously injuries and likely to die
- Critically Injured
Request Patrol Supervisor to respond if any of the above
Bicycle collision or Bicycle Collision and Pedestrian
Write “No Vehicle” on the TOP of the PAR, if handwritten copy prepared.
For Bicycle Collisions who notifies the friends/relatives if bicyclist or pedestrian is admitted to the hospital?

Vehicles will be D.A.R.P towed if the officers determines that the vehicle is unsafe to be driven EXCEPT
- Investigatory Evidence
- Franchise Tow
- Government Vehicles (May asked)
- RoTow (Stolen Vehicles)

- Fifteen thousand pounds or more (May) (Any vehicle)
For DARP what will the Patrol Supervisor do?
- Respond to the scene and supervise the removal
- Supervise the issuance of summonses to tow truck operators
- Ensure the preparation of “Report of Violation” when warranted.
What will the Desk Officer do for DARP Procedure?
- Process the “Report of Violation”
- Review, sign and assign a precinct serial number and forward to the Department of Consumer Affairs, Licensing Enforcement Division.

Vehicles leased or owned or rented and/or issued to the department by a federal, state, or city agency
Who is the Investigating Supervisor for Department Vehicle Collisions?
Precinct Commander Officer/Duty Captain- collision involves SPI or Death to Anyone
Patrol Supervisor, precinct of occurrence or a supervisor officer from the members command if available
What part of the Collision Report Police Department Vehicle need to be filled out by the Investigating Supervisor?
Fill out Part A
Department Vehicle Collisions- Patrol Supervisor
- Respond to the scene and ascertain the details of the collision
- Assume the role of the investigating Supervisor or request the response of the appropriate investigating supervisor
- Notify the DESK OFFICER of the details of the collision
- Notify OPERATIONS UNIT if a MOS is injured and requires hospital treatment.
——An additional notification to the Operation Unit is also required before final reports are prepared to UPDATE Operations of the status of the injured MOS.
Department Vehicle Collisions-Desk Officer
-Notify the Commanding Officer to respond to the scene, when applicable (Death or Serious Injury to ANYONE)
Notify the Patrol Borough of the collision AND
- Request the Duty Captain to respond when applicable (Commanding Officer Not Working) AND
- Enter the Borough Collision Number in the Telephone Record
- If the driver committed a motor vehicle infraction or fatality and operator is a Civilian MOS required to have a CDL the Desk Officer has to notify the Medical Division within 1 hour of the incident.
Medical Division is open 0700-1600 Monday-Friday. If the Medical Division is closed then contact the Medical Division Sick Desk and be guided by the location where to take the drug and alcoholic test .
Investigating Supervisor- Department Vehicle Collision
- Respond to the scene of the collision
- Conduct a canvass for witnesses and video surveillance that may have captured the collision.
- Interview all persons involved, including witnesses.
- Secure a copy of the video surveillance, if available
- Ensure a copy of the video is placed in a JEWELRY SECURITY ENVELOPE and vouchered as “INVESTIGATORY” evidence
- Select “MOS INVOLVED” caption - Enter in remarks “DO NOT DESTROY WITHOUT AUTHORIZATION OF THE COMMANDING OFFICER, LEGAL BUREAU”
- Notify the Precinct Detective Squad for assistance if video is available but no accessible due to technical difficulties
- If PDS can’t retrieve the video then notify Technical Assistance and Response Unit
Who prepares Part A of the Collision Report-Police Department Vehicle form?

- Make a preliminary determination and record the cause of collision on the Collision Report-Police Department Vehicle.
- Ensure that the cause of the collision is correctly identified and recorded on BOTH PAR and the Collision Report- Police Department Vehicle.
Who do you send a copy of the CR-PDV if a video was invoiced ?
Send copy of CR-PDV to the Commanding Officer, Risk Assessment Unit only if Video has been invoiced
Who does the MOS notify if they’re charged by their private insurance or their insurance has cancelled or not renewed because of a Department vehicle accident ?
- Notify the Commanding Officer who prepare a typed letterhead to the insurance company.
- UMOS is also to notify the New York State Department of Financial services
When will the Patrol Supervisor notify Operations Unit regarding Vehicle Collisions that resulted in death, Serious Physical Injury and likely to die , or Critical Injury?
