Comp 5 Flashcards

1
Q

Describe long lever ME for type 1 lumbar

A

Pt lateral recumbent with PTP up
Doc monitors apex of group curve with cephalad hand while flexing hips/knees to 90
Rotate pt’s top shoulder posterior to rotate spine through segment until motion is felt
Lift pt’s ankles toward ceiling to side bend lumbar spine into barrier
Pt pushes feet down toward floor

Q

Describe seated ME for type 1 lumbar

A

Student behind pt on opposite side of PTP
Pt places hand on side of PTP onto back of neck and grasps ipsilateral elbow with other hand
Student monitors at apex with one hand and uses other to grasp pt’s arm on side of dysfunction by passing under pt’s arm grasping elbow
Engage barrier by rotating and side bending pt
Pt tries to return trunk to neutral against resistance

Q

Describe long lever ME for flexion type 2 lumbar

A

Pt lateral recumbent with PTP down
Monitor segment with caudad hand
Grasp arm closest to table and pull anterior and superior to rotate spine to rotational barier
Switch monitoring hands
With caudad, flex both hips and knees to 90 degrees
Pt straightens lower leg and extends leg
St abducts top leg to address side bending
Pt adducts leg down against resistance

Q

Describe long lever ME for extension type 2 lumbar

A

Pt in modified Sims with PTP up
Doc monitors with cephalad hand
With caudad hand, flex both hips and knees through named segment
Lower pt’s legs off table to engage side bending barrier
Pt raises both ankles up toward ceiling against resistance

Q

Describe seated ME for type 2 lumbar

A

Student behind seated pt opposite side of dysfunction
Pt places hand on side of PTP on back of neck, and other hand grasps elbow
Student monitors PTP with one hand and grasp’s pt’s biceps by passing over pt’s arm grasping elbow
Engage flexion/extension, then rotation and side bending barriers
Pt tries to return trunk to neutral against resistance

Q

Describe HVLA for type 1 lumbar

A

Pt in lateral recumbent with PTP up
Doc monitor apex of group curve with caudad hand
Grasp arm closest to table and pull anterior through segment to engage rotation and cephalad to engage side bending barrier
Switch monitoring hands
Flex hips to 90 and have pt straighten lower leg and place top foot into opposite popliteal space (hook the foot)
use cephalad forearm against pt’s anterior shoulder
Caudad forearm against posterior aspect of pt’s hips
Induce posterior motion at pt’s shoulder while inducing anterior motion to hips to engage barrier
Pt exhales, and at end of exhalation, doc applies anterior rotational thrust with caudad forearm

Q

Describe HVLA for type 2 lumbar

A

Pt lateral recumbent with PTP up
Monitor with caudad hand
Grasp arm closest to table and pull anterior for rotational barrier and then caudad for side bending
Switch monitoring hands
Caudad hand flexes hips to 90, and pt straightens lower leg.
Put top leg into opposite popliteal space (hook the foot)
Cephalad forearm on anterior shoulder
Caudad forearm on posterior aspect of hips
Posterior force on shoulder and anterior force on hips
Apply anterior rotational thrust at end of pt exhalation

Q

Describe supine HVLA for type 1 lumbar (OB roll)

A

Pt supine with both hands behind neck
Student at opposite side of PTP
Monitor apex of curve and side bend trunk away until motion is felt
Reach across front of patient and place cephalad hand through space between arm and forearm so that dorsal aspect of hand is on pt’s manubrium
Caudad hand blocks linkage at ASIS on side of PTP
At end of exhalation, apply rotational thrust toward self through barrier with cephalad hand
