Compensatory Swallowing Maneuvers Flashcards
What are the 5 main compensatory Strategies
- Supraglottic Swallow
- Super-supraglottic swallow
- Effortful swallow
- Mendelsohn Maneuver
- Tongue hold maneuver (Masako maneuver)
Supraglottic Swallow (what is it)
4 steps
1. Inhale and hold breath
2. place bolus in swallow position
3. swallow while holding breath
4. cough after swallow before inhaling
When to use Supraglottic Swallow
- reduced or late vocal fold closure
- delayed pharyngeal swallow
Super-supraglottic swallow (what is it)
Same as supraglottic swallow with the addition of the instruction to bear down to increase false vocal fold closure and assist in the closing of posterior glottis
–> inhale and hold breath very tightly, bear down, maintain the hold, swallow, then cough
When to use super-supraglottic swallow
Reduced closure of airway entranced
Effortful swallow (what is it)
Squeeze hard with all your muscles while you swallow
When to use an effortful swallow
Reduced posterior movement of the tongue base
Mendelsohn Maneuver (what is it)
- patient initiates several dry swallows while trying to feel the thyroid prominence lift
- hold the thyroid up for several seconds
When to use the Mendelsohn Maneuver
- Reduced laryngeal movement
- Discoordinated swallow
Tongue Hold Maneuver (Masako)
Attempts to increase the pressures and time of contact of the tongue base to the pharyngeal wall
- hold the tongue between your front teeth and swallow
When to use the tongue hold maneuver (Masako)
lack of posterior pharyngeal wall contact with tongue
