What are the benefits and disadvantages of immediate dentures?

A

Benefit: physiological - avoid social embarrassment of no teeth

Disadvantage

  1. Time intensive
  2. Discomfort associated
  3. Won’t last forever - bone resorption

A

First 3-6 months

What are the different stages from dentate to providing CD?

A

Inadequate dentition

  1. Immediate complete –> perm complete denture
  2. Transitional partial –> transitional complete –> permanent complete
  3. Partial over denture –> complete over denture –> perm complete denture

Indications for immediate denture?

A

Remaining teeth health risk - infection/ radiotherapy
Fewer visits are essential - cost/time
Teeth misaligned/over erupted transitional impossible to provide
Hopeless prognosis remaining teeth

Contraindications immediate dentures?

A

The elderly
Large bony undercuts
Difficult extractions

Types of immediate dentures?

A

Labial flange
No labial flange

Conventional flange is ideal to aim for

When May socket-fit denture be used?

A

To fit into undercut

May find difficult to transition to labial flange if had socket-fit

What are significant factors when assessing pt for dentures?

A

Current partial dentures -use as transitional
Hx of nausea
Undercuts - if severe prolonged surgery will be needed

Warnings prior to tx?

A

Immediate denture not permanent - will need to be replaced when resorption has slowed down - will be further expense
Adequate emergency follow up - post insertion pain

Process of immediate denture?

A
  1. Impression taken and registration carried out
  2. Teeth cut from model at level w/ gingiva
  3. Region teeth removed - residual undercut and sharp edges removed
  4. Acrylic surgeons guide prepared
  5. XLA with minimal trauma
  6. Place surgeon guide over extraction socket - blanching on underlying issue - remove bone
  7. Insert denture and any defect occlusion corrected

How long should immediate be kept in following fit?

A

24hours

Come back review appt -remove, clean and refit

Program of review for immediate denture?

A

Day 1: adjust fitting surface and occlusion
1st week : correct pain, advise fixative if lack security
1st month: further correction and temporary soft lining
6 month: permanent reline
12 month: replace perm dentures
