Behavior of symptoms
Changes in location or intensity of the patient’s symptoms during various activities, positions, or periods of rest over a 24 hour.

Is there anything you can do to make your symptoms feel better?
Three components of irritability
How long can you do this activity or be in this position before your symptoms increase? When your symptoms are aggravated by this activity a position how long can you continue before you need to stop? Once you stop the activity how long does it take for your symptoms to settle?
Sleeping Qs
Trouble sleeping? Is it your symptoms that prevent you from getting to sleep? Or is that your symptoms that week you during the night? When you wake up night what do you do to go back to sleep? How long does it take you to get back to sleep? How frequent are you waking?
24 hour pattern questions
How are your symptoms when you first wake in the morning? How are your symptoms as you progress through the day?
Sustained pastors, movement from sustained postures, affects of rest periods
Severity
Clinicians assessment of the magnitude of the patient’s problem. Dictated by how symptoms influence functional activities.
History of current condition questions
When did this problem start? How did this problem start? Were there any changes?What did you do when this problem started? How are your symptoms now compared to when they first started? Have you had any treatment for this problem? How did you respond to treatment?
Nature
Clinicians assessment of the structure our pathology responsible for producing the impairment/symptoms for which the patient seeks physical therapy intervention. Contributing factors, psychosocial issues
Stage
Progression of symptoms for the current episode and progression of disorder and repeated episodes
Stability
Consistency of distribution of symptoms, movement or rest and capabilities, how predictable and consistent responses are to movements in their positions.
