How long are the Confined Space Risk Assessment Records kept for?
The MEO/EO/STO is responsible for retaining records of any Confined Space Risk Assessments that are conducted within the unit. Records of Confined Space Risk Assessments must be kept for at least 28 days after the work to which it relates is completed. If a notifiable incident has occurred during the activity, the Risk Assessment must be retained for two years.
How long before entry can a AC870 be completed
Hazards and risks within confined spaces can change quickly; as such, the Confined Space Entry Permit must be issued no more than one hour prior to the confined space entry. Should this time frame be exceeded, confined space mandatory controls, risks and hazards must be reassessed.
How long can a AC870 last for?
Confined Space Entry Permit will last for a total of 10 hours. Once 10 hours have elapsed, the Confined Space Entry Permit may be reissued for a further 10 hours. The maximum life of the Confined Space Entry Permit (including after reissue) is 20 hours.

All confirmed and approved Confined Space Entry Permits must be registered in the Confined Space Entry Permit Register.
If a fan is being used to ventilate a confined space, how far should it be from the confined space
If a portable fan is used for force ventilation, the fan must be seven metres from the opening and the discharge hose lowered to near the bottom of the confined compartment.
What are some examples of confined spaces
a. fuel tanks (diesoline, gasoline, kerosene, AVCAT) or any other liquid or gaseous fuel;
b. water tanks (feed, fresh, ballast), void spaces, cofferdams, double bottom/wing compartments, sewage treatment tanks (CHT systems);
c. oil tanks (sludge oil, lubricating, hydraulic);
d. cable lockers (anchor cable stowage);
e. open-topped spaces such as pits or degreasers, pipes, sewers, shafts, trunking ducts and similar structures; and
f. gas turbine rotor casing, piston engine enclosed crankcase, main circulating water inlet and discharge pipes, pressure vessels
What are the responsibilities of a confined space sentry

What is a confined space
- A space not designed or intended to be occupied by a person
- Is designed to be a normal atmospheric pressure
- Is likely to cause a risk to health and safety from
a. An atmosphere that does not have a safe oxygen level
b. Contaminants including airborne gases, vapours and dusts that may cause injury from fire or explosion
c. Harmful concentrations of any airborne contaminants
d. Engulfment

When can a Confined Space Entry Permit be issued?
A confined space entry permit can only be issued no more than one hour prior to the confined space entry
When does a confined entry become void?
A Confined Space Entry Permit becomes void if any of the following occurs:
a. after 10 hours have elapsed from time of issue, unless reissue authorised then after 20 hours from time of issue (noting only one reissue is allowed);
b. when gas detection equipment (or other device) indicates that oxygen or atmosphere levels are unsafe; in this circumstance, don Emergency Life Supporting Respiratory Device (ELSRD) or Open Circuit Compressed Air Breathing Apparatus (OCCABA) and vacate the compartment/area and raise the alarm;
c. a new hazard or risk is introduced to the confined space (such as the introduction of additional or new chemicals not listed on the Confined Spaces Entry Permit, or the change of the use of equipment) which alters the risk assessment;
d. if the CSEPAA listed on the Confined Space Entry Permit, or CS Supervisor, changes;
e. if there is a change to the nature of the work that is being performed within the confined space; and
f. if there is a significant change in the confined space atmosphere.
When should OCCACAs be used in confined spaces?
Navy requires OCCABAs to be used when confined spaces are entered in any of the following circumstances:
1. after the authorisation of a risk assessment for the confined space to conduct initial testing or monitoring;
2. following the issue of a Confined Space Entry Permit (Form AC870) where breathing equipment is required by the Confined Space Risk Assessment (Form AC869); and
3. to rescue someone from a confined space.
Where should the AC870 be placed after completion?
Confined Space Entry Permit (Form AC870) has been issued no more than one hour prior to entry and is displayed at a prominent place near the confined space.
