Signs/Symptoms of congenital dislocated hip
restricted abduction
anchor: asymmetric thigh folds (more folds on the dislocated side) he sign is more reliable from 4 months to 18 months. It is less reliable after 18 months.
relative femoral shortening limp:due to the position of the femur head (up and out).
Positive trendelenburg test
externally rotated foot
waddling gait
hips and knees flexed to 90 deg and then abducting thigh while stabilizing pelvis. Test is positive when palpable click is felt as femoral head is made to enter acetabulum
A. Ortolani Test
Look at the level of the knees when knees and hips are flexed. The height of the knee on the affected side will appear lower.
Galeazzi Test (also called Allis sign)
when the child stands on the affected side, the opposite (normal) hip will drop down due to weakness of the abductors on the affected side.
D. Trendelenburg Test
baby prone with more glueteal folds on the dislocated side
