State the key counselling points for patient starting on Warfarin (7)
1) Tablet brand and tell patient that the brand is non-interchangeable due to narrow Therapeutic index
2) Note the dates of acute illnesses (e.g N/V/D, fever) before next TCU
* Especially if close to next blood draw -> Prevents unnecessary dose adjustment
3) Tell patient to take precautions to prevent injury (e.g high contact sports)
4) For those planning pregnancy should stop warfarin and start LMWH
5) For those breastfeeding, should not be on warfarin
6) Keep Vit K intake consistent
7) Action and Indication for Warfarin
State the counselling points for patient with stent insertion (4)
1) Explain thrombogenicity of cardiac stents (you need time to heal, and body does not like foreign agent)
2) DAPT complementary (need to take both concurrently); why need to be adherent
3) Duration of DAPT, aspirin usually lifelong
4) How to manage when invasive procedures required, e.g. dental extraction, scopes
-> Contact primary cardiologist to see if they can hold off; usually for elective procedure, DAPT will NOT be stopped (usually only stopped in emergencies)
