Optic - sight
Monocular blindness, loss of pupillary constriction, absence of blink reflex
Cranial nerve I
Olfactory - smell
Anosmia (loss of smell)

Oculomotor - muscles of the eye
Ptosis, loss of pupil dilation, loss of accommodation of light reflex
IV
Trochlear - muscles of the eye
Diplopia, failure to rotate eye up and out
V
Trigeminal - sensory for face, muscles for chewing
Loss of facial sensation, weakness of the muscles of mastication, deviation of opened jaw to ipsilateral side.
VI
Abducens - muscles of the eye
Diplopia, convergent squint and abductor paralysis of ipsilateral eye
VII
Facial - muscles of face, taste anterior 2/3 of tongue
Ipsilateral facial paralysis, dry mouth, loss of taste anterior 2/3 of the tongue (Bell’s palsy)
VIII
Vestibulocochlear (auditory) - balance and hearing
Vertigo, nystagmus(rapid mov of eye), dysequilibrium, tinnitus, loss of hearing
IX
Glossopharyngeal - taste posterior 1/3 of the tongue, muscles of pharynx
Slight dysphagia, partial dry mouth, loss of taste posterior third of tongue
X
Vagus - internal organs
Palpitations, tachycardia, vomiting, slowing of respiration, ipsilateral paralysis of soft palate and larynx, hoarseness, anesthesia of the larynx
XI
Spinal accessory - SCM and trapezius
Weakness in shrugging ipsilateral shoulder, turning head to opposite side
XII
Hypoglossal - muscles of the tongue
Unilateral paralysis of the tongue
