Identify hazards

A

A hazard is any condition with potential to negatively impact the mission, cause injury, death, or property damage. 3 actions are analyze mission, list hazards, and determine hazard’s root cause.

Assess hazards

A

The associated degree of risk in terms of probability and severity must be determined from all hazards identified. A prioritized list is created according to severity, probability, risk assessment codes and pitfalls.

A

Does the benefit outweigh the cost? Identify control options per hazard, determine the effect of those hazards, and decide how to proceed.

A

A

A

Risk assessment involves setting a risk assessment code for each hazard. Probability and severity codes are given 1-4 and a-d then used in the risk assessment matrix to get each RAC.

A

Any training that exposes staff/students to potential risks of death, disability, or loss during training. Risk assessment codes are used to determine high risk per hazards identified.

Discuss the difference between voluntary and involuntary types of courses.

A

Voluntary is when sailor voluntary enrolled and can drop on request while still being able to return to original command.
Involuntary is training where sailor is enrolled through accession or follow on specialty skill.

Discuss what Drop on Request “DOR” is and the procedures for conducting one.

A

When a student is enrolled in a voluntary status all they have to do is make their intentions known to drop. Immediately the student should be removed from the course and a written summary of course of action that was taken must be entered into service record as well as copy to remain in command’s permanent files.

A

is used in any situation where students or staff have concern for personal safety or need to clarify procedures or requirements. Training will stop and resume upon the situation returning to safe.

Discuss the purpose of the Emergency Action Plan (EAP) and how often must it be reviewed for accuracy of information. Identify the periodic requirements for fully exercising the EAP. How often is it to be fully exercised?

A

It’s checklist of responses and actions required to aid and extract mishap victims. The EAP will contain emergercy contact numbers, emergency muster locations, shutdown procedures, and emergency equipment locations. A walkthrough will be conducted every quarter as well as full excercise and validation of the plan on an annual basis.
