What is depression?
consistent mood lowering and a lack of interest in previously rewarding or enjoyable activities
What are the symptoms of depression:
- negative views
- worthlessness
- guilt
- sleep disturbances
- loss of energy
- impaired concentration
What are the consequences of depression?
- reduce quality of life
- impaired work ability
- poor social functioning
- suicide
- thoughts of suicide
- pessimism
What are the vulnerabilities of depression?
- losses
- stressful life events
- lack of social support
- familial factors
- genetic factors
What medications impact mood?
- beta blockers
- hormonal treatments
- steroids
What is the sub threshold severity description?
2-4 symptoms of depression
what is mild depression?
5 or more symptoms with minor functional impairement
what is moderate depression?
symptoms or functional impairment in between mild and severe
What is severe depression
- most symptoms and interfere with functioning
- can occur with or without psychotic symptoms
What is persistent sub threshold depressive symtpoms
sub threshold symptoms for more days than not for at least two years which isn’t the consequence of partially resolved major depression
What is seasonal affective disorder?
episodes of depression which recur annually at the same time each year with remission in between

What is the patient health questionnaire 9 grading?
- mild: 5-10
- moderate: 10-15
- moderately severe: 15-19
- severe depression: 20-27
What do you prescribe for first episode of depression
- SSRI’s e.g. fluoxetine
Why to avoid tricyclics and MAO’s?
higher toxicity in overdose
- fatal CV effects
- overdose can cause sedation/coma/seizures
What is monitored?
- suicidal thoughts and attempts?
- cardiac disorders
- hypertension
- hyponatraemia
What are the low-intensivity psychosocial interventions?
- individual guided self-help
- computerised cognitive behavioural therapy
- group based CBT
What is high-intensity psychosocial interventions?
- generally reserved for people with moderate to severe depression
- individual CBT
- behavioural activation
What is counselling
- for people who decline antidepressants and high intensity psychological interventions
- uncertain long term benefit

amitriptyline
