What is a lotion?
Powder in water or oil suspension
What is a solution?
Medicine dissolved into solvent. Evaporates more quickly than other topical delivery methods.
What is a cream?
Water in oil emulsion
What is an ointment?
Oil in water emulsion. Evaporates much more slowly than other types of topical administrations.
Polymyxin B + Neomycin (Neosporin)
Topical antibacterial.
Used for superficial skin infx, otitis externa, prophylactically against infx. Disrupts bacterial cell wall and membranes. May be formulated with corticosteroids.
SFx: Irritation. Neomycin is ototoxic; do not use on punctured TM.
Route: Topical cream, lotion, ointment
Mupirocin (Bactroban)
Topical antibacterial. Especially active against staph and strep.
Used for impetigo, topical MRSA. Intranasal use for MRSA pts.
SFx: Irritation, burning sensation.
Ketoconazole (Nizarol)
Topical antifungal. Inhibits ergosterol in fungal wall synthesis.
Used for superficial fungal infection (most tineas), seborrheic dermatitis, topical yeast infx.
SFx: Skin irritation.
Route: Lotion, ointment or shampoo
Glucocortocoids
Anti-inflammatory agents used for: Atopic dermatitis, Contact/ allergic dermatitis, Psoriasis, Eczema, bullous diseases such as pemphigus, collagen vascular diseases such as SLE and granulomatous disease such as sarcoidosis.
SFx: Skin atrophy, Striae, Telangiectasias, Purpura, Acneiform lesions, Perioral dermatitis, Overgrowth of skin fungus and bacteria, Hypopigmentation, Rosacea.
Hydrocortisone (Cortef)
Glucocorticoid
Used for a variety of skin conditions
SFx: Skin thinning, hypopigmentation
Route: Topical, but may be used IV or IM as well. 1% or halogenated versions are Rx only.
Retinoids
Natural compounds and synthetic derivatives of retinol that exhibit vitamin A activity. Affect skin in a variety of ways:
Cellular proliferation and differentiation, immune function, inflammation and sebum production may all be modified by retinoid treatment.
Used for: Cystic and papular acne, basal cell cancer, squamous cell cancer, actinic keratosis, psoriasis, and cutaneous aging
Tretinoin (Retin A)
Retinoid (Vit A deriviative)
Used for: Cystic acne, photo-damaged skin
SFx: Erythema, peeling, burning and stinging. Photosensivity.
Topical, applied once per day. May take months to see clinical results.
Isoretinoin (Accutane)
Retinoid.
Used for severe cystic acne, rosacea, hidradenitis supperativa.
Topical.
SFx: Teratogen, depression, psychosis, and on rare occasions suicidal ideation, suicide attempts and suicide, HA, myalgias, arthralgias, hyperlipidemia, fatty liver, hepatitis and pancreatitis.
CAT X - absolute contraindication. Pt must show evidence of using two types of birth control before Rx may be written. NO REFILLS.
Psoriasis tx?
Stepwise progression of: Topical corticosteroids Topical Vit. D Phototherapy Systemic Therapy (oral steroids, chemo agents, TNF inhibitors)
PUVA therapy
Stands for:
Psoralen Ultraviolet A therapy. Pt applies a psoralen agent and is exposed to UVA light source. MOA unknown but seems to have anti-inflammatory action and inhibits DNA synthesis.
SFx: Nausea, painful erethema, blistering, premature aging of skin.
