Acarbose
DPP-4
Oral agents to use with:
Insulin Resistance (esp in muscle and fat)
Metformin
Thiazolidinediones
Metformin is the first choice for oral agents for DM2 (minimizes wt gain, reduces insulin resistance) but if need additional glycemic control can add:
Sulfonylurea or incretin mimetic
or other oral agent for 2 or 3 drug combos
Start insulin therapy in patients who:
Have an HgbA1c that approaches 8% in spite of optimal oral therapy
May increase beta cell mass
Gets beta cells to only release insulin if BG is elevated
Byetta/Exenatide
-Give to DM2 if unable to control on other stuff
Might give them pancreatitis tho.
