Case prioritisation

A
  • Based on D/C, risk of deterioration, treatment protocols
  • Examples of Cat 1 may include resp. distress, D/C expected later in day, recommended day 1 visit post THR/TKR etc.
  • Examples of lower Cat may include mobility check, D/C in upcoming days etc.

A
  • Aim to reduce LOS, prevent readmissions, improve service co-ord, contain costs
  • D/C to enable hospital D/C or D/C from physio despite ongoing inpatient care
  • D/C decisions involve: assessments, care plan development, implementation and follow up

  • Outcome measures?
  • Establish baselines and goals required for D/C
  • Social and physical factors
  • Determine appropriate pathways e.g. home, home + support, transfer to other facility
  • Prepare, educate and communicate the care plan
  • Handover clinical information- ISBAR

  • S = Situation- what’s going on with the patient
  • B = Background- clinical background/context
