2x maintenance dose (8-4-2)

Maintenance dose = (4-2-1 rule) 4mL/kg first 10kg, 2mL/kg next 10 kg, 1mL/kg for rest of weight

Generally they are hypo K systemically, with transient hyper K in the acute phase

K<3.3: hold insulin give 20-40mEq K/hr
K>3.3 but <5.3: continue insulin gtt, supplement IVF w 20-30 mEq K/1L fluid
