Dopamine (therapeutic dosage range and responses)
• 2.0 to 5.0 mcg/kg/min: negligible beta-adrenergic responses
• 5.0 to 10 mcg/kg/min: beta-adrenergic responses (positive chronotropic and inotropic effects)
• 10 to 20 mcg/kg/min: alpha-adrenergic responses (vasoconstriction, increased blood pressure and increased preload)
• > 20 mcg/kg/min: may increase the heart rate and myocardial oxygen demand to undesirable limits.
Acetaminophen (adult dose)
325 to 1,000 mg every 4 to 6 hours
Maximum 4.0 g daily
Acetylsalicylic acid (ASA) adult dose
160 to 325mg
Activated charcoal (adult dose)
30 to 60gm (1.0 to 2.0 gm/kg)
Adenosine (adult dose)
6.0mg RAPID bolus over 1 to 3 seconds follow by 20 cc normal saline flush
If no response to the initial bolus after 1 to 2 minutes, administer 12 mg RAPID bolus over 1 to 3 seconds followed by 20 cc normal saline flush
Albuterol sulfate (adult dose)
• 2.5mg in 3.0mL normal saline (unit dose) via SVN
• 2.5mg in 3.0mL normal saline (unit dose) via in-line SVN with a bag-valve mask
• May repeat the dosage
Amiodarone (adult dose)
• Cardiac arrest: 300mg
• If no response, administer 150 mg over 3 to 5 minutes
• Tachydysrthymias: 150mg in 50mL D5W infused over 10 minutes
• Infusion: 1.0mg/min
Atropine sulfate (adult dose)
• Hemodynamically significant bradycardia: 1.0mg
• May repeat the dosage every 3 to 5 minutes to maximum dosage 3.0mg
• Organophosphate poisoning/carbamate poisoning/nerve agent exposure: 1.0 to 4.0mg
• May double the dosage every 5 minutes
• ET: 2.0 to 2.5mg diluted in 10mL normal saline
Bumetanide (adult dose)
0.5 to 1.0mg
Calcium chloride (adult dose)
• 500 to 1,000mg (0.5 to 1.0gm)
• May repeat the dosage in 10 minutes
Calcium gluconate 2.5% topical gel (adult dose)
If possible, patient should wash affected area & massage the gel into the skin
Calcium gluconate (adult dose)
• Hyperkalemia: 1.0 gm
• May repeat the dosage
Dexamethasone (adult dose)
8.0 to 24mg
Dextrose 50% (adult dose)
12.5 to 25gm
Diazepam (adult dose)
2.0mg increments over 2 minutes
Diltiazem (adult dose)
• 0.25mg/kg (usually 20mg) over 2 minutes
• May repeat the dosage 0.35 mg/kg (usually 25mg) in 15 minutes
• Infusion: 5.0 to 15 mg/hour
Diphenhydramine (adult dose)
• Allergic reaction/anaphylaxis: 25 to 50mg
• Phenothiazine reaction with extrapyramidal symptoms: 10 to 50mg
Dopamine (adult dose)
Infusion: 2.0 to 20 mcg/kg/min (reference the Dopamine Dosage Chart)
Epinephrine (adult dosages)
• Cardiac arrest: 1.0 mg (1:10,000) IV/IO
• May repeat the dosage every 3 to 5 minutes during resuscitation
• Cardiac arrest: 2.0 to 2.5 mg (1:1,000) ET diluted in 10mL normal saline
• May repeat the dosage every 3 to 5 minutes during resuscitation
• Hemodynamically significant bradycardia: 2.0 to 10.0 mcg/min infusion
• Hemodynamically significant hypotension: 2.0 to 10.0 mcg/min infusion
• Bronchospasm associated with chronic bronchitis and emphysema/allergic reaction/anaphylaxis : 0.3 to 0.5 mg (1:1,000) IM Adult EpiPen auto injector 0.3mg IM
Etomidate (adult dose)
0.2 to 0.6 mg/kg (usually 20mg) over 30 to 60 seconds
Fentanyl (adult dose)
• 25 to 50 mcg
• Max dosage 200 mcg
Furosemide (adult dose)
• If the patient is prescribed furosemide: the initial dosage is 2 times the patient’s total daily prescribed dosage of the drug (usually 40 to 80 mg)
• If the patient is not prescribed furosemide: 0.5 to 1.0 mg/kg (usually 30 to 40 mg)
Glucagon (adult dose)
• 1.0 mg
• May repeat the dosage in 7 to 10 minutes
Hydroxocobalamin (adult dose)
• 5.0 gm over 15 minutes
