Q

What do you check for in the objective exam before getting behind the wheel for training?

A
  • vision + vestibular stuff
  • cognition
  • UE strength/function
  • neck ROM!!! (DMD, SCI)
  • perception
  • sensation
  • coordination
  • balance

9
Q

Why is it better to transfer into the seat than use your power chair?

A

Factory seat (transfer into seat) is safer than power chair
* can get an add on to allow seat to raise/lower for downhill transfers
* if you can transfer, preferred

Q

can you drive from a scooter? what about power WC?

A
  • cannot drive with scooter (unsafe)
  • power WC that rise/lower are still unstable in vehicle but you can.
  • manual WC: frames are not made for this –> put artificial back for support and HEAD REST –> rigidize the folding base (adds 10 lbs)

11
Q

its easier to teach someone who knew how to drive before than someone brand new.
EXCEPT for using what?

A

LEFT FOOT ACCELERATOR
* pedal on left side
* brake in middle
* COGNITIVE (mm memory problem)

13
Q

You can teach someone to press accelerator with left foot, brake with right foot. Why would you do this?

A
  • sensation has to be normal (feedback is important)
  • good for MS, other conditions where foot switching is slow
*if reaction time is SLOW/mvmt of foot is slow

Q

What are visual field requirements for TEXAS?
If you have homonymous hemianopsia, can you drive?

A

140 degrees of uninterrupted visual field
*if you have 1 blind eye, you can meet this requirement (150 ish visual field)
*if you have homonymous hemianopsia, probably NO–> RARE if someone can compensate and qualify with good tracking

can drive within restrictions if u dont pass visual requirements

Q

Bells test is for…

A

of bells + time

visual neglect and hemispace affected

*scanning test (cross out every bell you see –> kinda like eye spy)
TIMED

18
Q

What is the max score for Bell’s test?

19
Q

What indicates USN on the Bell’s test?
how long should it take?

A

USN (Unilateral spatial neglect): omission of 6 or more bells on the right or left half

*timed less than 5 minutes

20
Q

what is the trail making test part A or B

A

connect all the circles in number order
*part B is also incorporating multitasking

TIMED

21
Q

scoring for Trail A and B test

A

higher score = greater impairment

Trail A average: 29 seconds
Trail B average: 75 seconds

fail for A: over 78 seconds
fail for B: over 273 seconds

22
Q

Do you need to check sign understanding?
What about map reading?

A

YES
YES –> do you have perspective to know where you are or how to get home? TOPIGRAPHICAL ORIENTATION IS SO IMPORTANT
*can you read the small print too?

23
Q

How long should the sign identification test take?

A

timed test: 90 seconds or less

24
Q

How long should map reading timed test take?

A

150 seconds or less

25
What are the other perceptual tests we didn't review in class
1. visual closure 2. Driver performance Analysis System (DPAS) --> looks like assesses perceptual skills and traffic risk 3. BCAT (cognitive test) 4. reaction time (simple brake reaction time)
26
What is a normal BCAT score and what indicates MCI vs mild dementia?
normal: 44-50 Mild cognitive impairment: 34-43 mild dementia: 25-33 mod-severe dementia: 0-24
27
What does the physical assessment consist of?
* TRANSFER LEVEL * ability to load WC * UE/LE MMT, ROM, endurance
28
If you can't load your WC independently, what can you use?
chair topper WC loading device
29
What is an adapt solutions XL seat
like a powered transfer board *helps w/ height descrepancy if transfers are iffy
30
What is a truck lift option?
camber shell powered with hoyer lift (opens up, lift in, closes, then puts WC into back of truck) but weight limit (200-300 lbs)
31
what are other things you need to practice
pumping gas! by law, should be helped if you need help with pumping gas, but if a small gas station they won't.
32
behind the wheel assessment: 5 first things to check
1. simple tasks 2. stops/starts 3. ability to turn wheel 4. ability to operate ignition, gear selector, etc (static secondary controls) 5. ability to operate in motion controls (turn signals)
33
Can you recommend someone to retire from driving?
In TX, can report someone anonymously to Medical Advisory Board if you think they should be evaluated for driving --> make you take DPS test, will talk to ur doc *but need to communicate w patient bc they can figure out
34
behind the wheel assessment: next 4 things to check
6. complex tasks 7. changing lanes under pressure 8. finding specific location 9. follow 2-3 step directions
35
tri pin
need wrist function steer with shoulder *C5-6 can use this
36
v grip
need wrist stability
37
palmar steering device
arthritis, can steer with shoulder
38
how many functions can you do simultaneously? if you get mixed up, can be like pushing gas and brake at same time. power control joystick: motor does the driving (nothing providing resistance)
39
what are 5 steering device options
1. spinner knob 2. tri pin 3. amputee ring 4. palmar steering device 5. v-grip
40
What are hand control options
push/pull push/right angle push/rock push/twist
41
Can you get a pedal extension to make it closer
yes
