Epinephrine pedi doses
CA- 1:10000 0.01mg/kg or 0.1 mL/kg @ 3-5 min IV/IO RAD/anaphylaxis 1:1000 0.01 mg/kg IM max 0.3 mg or 0.01 mg/kg in 100 mL NS slow IV max 0.3 mg Bradycardia/Shock 1:1000 0.1-1.0 mcg/kg/minIV
Racemic Epinephrine dosage
Pedi only
0.25-0.75 mL of 2.25% solution small vol neb to total of 3 mL
Dexamethasone dose
A and P:
0.6 mg/kg IV/IO/IM/PO to max 10 mg
Methylergonovine doses
Adult only:
0.2 mg IV/IO/IM @ 2-4 hours to max 1 mg
Oxytocin dose
Adult only:
3-10 Units IM
Or
10-20 units in 1 L NS IV/IO @ 20-40 mL/hr titrated to bleed
Terbutaline dosage
Adult only:
0.25 mg SQ @ 15-30 min to max 0.5 mg
Lorazepam doses
Adult:
2-4 mg IV/IO/IM @ 5-10 min to max of 8 mg
Pedi:
0.05-0.20 mg/kg IV/IO/IM @ 5-10 min to max 0.2 mg/kg
Diazepam doses
Adult:
5-15 mg IV/IO/IM @ 5-10 min to max 30 mg
Pedi:
0.2-0.5 mg/kg IV/IO/IM @ 5-10 min to max 10 mg/dose
Midazolam doses
Adult:
2-10 mg IV/IO/IM @ 5-10 min
Pedi:
0.1 mg/kg to max 10 mg IV/IO/IM @ 5-10 min
Nitroglycerin doses
Adult only:
ACS:
0.4 mg SL @ 3-5 min Max 3 doses
CHF:
0.4-0.8 mg SL @ 3-5 min- if effective, 0.5”-2” dermal paste
IV infusion: begin at 10 mcg/min increased by 10 mcg/min @ 3-5 min until effect-max 200 mcg/min
Albuterol dose
A and P:
2.5 mg in 2.5 mL NS inhaled
Levalbuterol doses
Adult:
0.63 mg in 3 mL NS inhaled
Pedi:
0.31 mg in 3 mL NS inhaled
Ipratropium Bromide dose
A and P:
0.5 mg inhalation (added to albuterol)
Methylprednisolon dose
Adult:
125 mg IV/IO
Pedi:
2 mg/kg IV/IO
Morphine Sulfate dose
A and P:
0.1 mg/kg to max 10 mg/dose @ 5-10 min IV/IO/IM
Fentanyl doses
A and P:
1 mcg/kg max 100 mcg/dose @ 5-10 min IV/IO/IM/IN
Hydromorphone dose
Adult:
0.5-2.0 mg IV/IO
Or
2-4 mg IM
Pedi:
0.015 mg/kg/dose IV
Ondansetron dose
Adult:
4 mg repeated x 1 IV/IO/IM/PO
Pedi:
0.15 mg/kg repeated x1 IV/IO/IM/PO
Adenosine doses
Adult:
6 mg rapid IV push followed by 20 mL NS flush
12 mg rapid IV push followed by 20 mL NS flush
Pedi:
- 1 mg/kg max of 6 mg rapid IV push followed by 5-10 mL NS flush
- 2 mg/kg max 12 mg rapid IV push followed by 5-10 mL NS flush
Atropine doses
Adult:
Bradycardia: 0.5 mg @3-5 min to max 3 mg IV/IO
Organophosphate poisoning: 2-5 mg as necessary
Pedi:
Brady: 0.02 mg/kg ( min 0.1 mg-max 0.5 mg) IV/IO x 1
Diltiazem dose
Adult only:
Initial: 0.25 mg/kg over 2 minutes (15-20 mg) IV/IO
Second (after 15 min): 0.35 mg/kg over 2 min (20-25 mg)
Maintenance infusion: 5-15 mg/hr titrated to heart rate
