Adult: (pulseless VF/VT) IV initial bolus of 1.0-1.5 mg/kg every 3-5 min, max 3 mg/kg, then maintenance drip of 2-4 mg/min
(antidysrhythmic) Boluses of 0.5-0.75 mg/kg every 5-10 min, max 3 mg/kg, with return of rhythm, drip at 2-4 mg/min
(IO pain mgmt) 20-40 mg, very slow IO push

Ped: 1 mg/kg, repeat max 1 time in 3-5 min for pulseless VF/VT or in 15 min for dysrhythmias
(IO pain mgmt) 0.1 mg/kg, very slow IO push
