Morphine
morphine sulfate, 2-4mg IV push every 2 to 3 hours as needed for relief of severe pain
Hydrcodone/acetaminophen
5mg/325mg, 1-2 tablets every 4 to 6 hours for relief of moderate pain
Acetaminophen
325 to 650mg every 4 to 6 hours for relief of mild to moderate pain
NSAID
Aspirin, 325mg every 4 hours as needed for relief of pain
Mu receptor antagonist
Tramadol, 50 to 100mg every 6 hours as needed for relief of moderate pain
Mixed agonist/antagonist
Nalbuphine 10mg IV push every 6 hours for relief of moderate pain
Pregabalin
75mg one capsule twice a day
Duloxetine
60mg one capsule once a day
PPI
Omeprazole, 20mg, one capsule once daily
H2 receptor antagonist
Famotidine, 20mg, one tablet twice a day
gastric protectant
Sucralfate, 1g, Take 4 times a day on an empty stomach for 4 to 8 weeks
antacid
Aluminum/magnesium hydroxide suspension, 30ml every 4 hours as needed for epigastric pain relief
Prostaglandin
cytotec, 200mcg, one tablet 4 times a day with food
Prokinetics
Metoclopramide, 10mg one tablet 4 times a day 30 minutes before meals and at bedtime
tylenol routine use in OA
325mg 3-4x a day
Naproxen chronic use in OA
500mg bid
fentanyl patch
(blank) mcg per hour, reapply path every 3 days for persistent moderate to severe pain, max dose 300mcg per hour
hydroxychloroquine
400mg, once daily
sulfasalazine
sulfasalazine enteric coated, 500mg, take two tablets twice a day
Methotrexate
10mg orally, once a week
leflunomide
20mg, once daily
loading dose for leflunomide
100mg for 1st 3 days of tx, then 20mg once daily
Naproxen chronic use in OA
500mg every 12 hours
ibuprofen
200-400mg every 4 to 6 hours
