What is a DVT
Development of a thrombus in the deep veins of the lower extremities or pelvis.
Risk factors for a DVT?
Prolonged immobilization Bed rest > 3 days Post op Hx of a DVT Obesity age Trauma Malignancy Pregnancy and postpartum Stroke DM,COPD, Factor 5 Leiden ( genetics)
Symptoms
Asymptomatic Pain and swelling in the affected limb erythema tenderness leg pain with dorsi flexion
Common differentials with a DVT
Cellulitis, thrombophlebitis, Chronic venous insufficiency, Lymphedema, Bakers cyst, Knee joint pain, Hematoma, muscle injury.
What are the 2 categories of DVT
Distal
Proximal
Explain the 2 categories of a DVT
Distal: confined to the deep calf vein.
Proximal: involves the popliteal, femoral or iliac veins ( most troublesome)
How do you diagnosis a DVT
High D-dimer , must perform the following test,
Lower or comprehensive ( whole leg) ultrasound.
What is a probability test for a DVT
Wells score
What are the score values for a Wells score
High probability >3, moderate 1-2, low 0
what is the role of anticoagulation
prevent clot extension
