Symptomatic Sinus Brady:
- Atropine
- epi
- temp pace maker
(have crash cart at bedside)
Sinus Tachy:
amiodarone, adenosine, increase fluids, abolish cause
Atrial Flutter:
digoxin, dilitizam (CCB)
atrial flutter unstable:
cardioversion
Atrial Fib:
more than 48 hours TEE, less than 48 cardioversion
Atrial fib meds:
heparin, Coumadin
PAC:
nothing; adjust meds, abolish cause, only if more than 6 to worry
SVT:
CCB, adenosine, breath holding
PVC:
amiodarone, lidocaine, sotalol, give drip after bolus or can go back into bad rhythm
v-tach stable WITH PULSE:
12 lead, procainamide, amioadarone
v-tach UNSTABLE WITH pulse:
cardioversion
v-tach unstable WITHOUT pulse:
CPR!, defibrillator, precordial pump, ICD, IV mag if torsades, amiodarone
V-fib:
CPR until defibrillator arrives, epinephrine 3-5mins, ONE does of vasopressin, intubation, amiodarone/lidocaine drip
Asystole:
epi, vasopressin, CPR
First degree block:
monitor symptoms
second degree 1:
monitor, atropine
second degree 2:
pacemaker
type 3:
pacemaker
