History of cerebral haemorrhage
Recent myocardial infarction (less than 3 months)
Aortic aneurysm
Uncontrolled cardiac arrhythmias
Decompensated cardiac failure (ECHO may be helpful)
Acute respiratory infection
Deep vein thrombosis
