thickening of edema due to lack of vascularization
what is the status of edema in the inflammatory phase?
lots of edema

what is the status of edema in the fibroblastic phase?
viscous edema which is more difficult to push out
what is the status of edema in the maturation phase?
fibrosis/CT forming which can lead to brawny edema
what is pitting edema?
when external pressure leaves depression in the tissue
becoming gel-like
what are traditional techniques to reduce edema?
elevation
AROM
retrograde massage
where should pt be elevated for edema?
heart level or above
except with replanted digit/limb
how do you perform retrograde massage for fibroblastic edema?
hand to forearm then empty
fingers to forearm
how is coban wrap used?
used to maintain what was lost
distal to proximal
shrinks with time
how are isotoner gloves used?
gives light pressure all over
how are contrast baths used for edema?
should be used at tap temp warm/cold
NOT for circulation comorbidities
when is Manual edema mobilization used?
fibrotic stage edema
better than retrograde massage
too good for acute
what does MEM include?
exercises
light skin traction
massage techniques following lymph paths
low compression garments
what is the ideal pt for MEM?
healthy lymph system
active participant
performs some independently btwn sessions
what are contraindications for MEM?
inflammation, open wound, blood clot, cancer, cardiac issues, renal failure etc.
what is lymphedema?
protein rich lymphatic fluid accumulation caused by interruption of lymph flow
“more going in than out”
chronic + incurable
what is the primary type of lymphedema?
unknown cause
results from lymphatic dysplasia
can be from birth
can develop later
what is secondary type of lymphedema?
more common
cause by surgery, radiation, injury, trauma, scarring, infection
what are the sys of lymphedema?
swelling, taut skin, heaviness, tingling, pitting edema, stemmers sign, deepened folds on toes/fingers, hardening, reddening
what are the txs for lymphedema?
manual lymphatic drainage (MLD)
complete decongestive therapy (CDT)
what is MLD?
gentle manual techniques to activate lymph and reroute to different system
used with other modalities
what is CDT?
most successful for chronic lymedema
MLD + bandaging + exercise + skin care + nail care
