When would you reduce the dose to 30mg OD?

A

If one or more of the following are present:

  • Moderate or severe renal impairment (creatinine clearance [CrCL] 15–50 mL/min).
  • Body weight of 60 kg or less.
  • The person is receiving concomitant treatment with any of following P-glycoprotein (P-gp) inhibitors: ciclosporin, dronedarone, erythromycin, or ketoconazole.

  • For people taking 60 mg of edoxaban, prescribe 30 mg once daily with warfarin.
  • For people taking 30 mg of edoxaban, prescribe 15 mg once daily with warfarin.
