Narrow QRS w/ no P wave or upside down P wave
Multifocal Atrial Tachycardia
Rapid Irregular rhythm > 100bpm; at least 3 distinct P wave morphologies
Atrial Flutter
Regular, Sawtooth pattern, Narrow QRS
Atrial Fibrillation
No discernable P waves; irregularly irregular pattern of QRS complexes
Right BBB
V1: Wide upward RsR’
V5/V6: Slurred S wave
Left BBB
V1: Wide downward QRS
V5/V6: Double notch
Right Ventricular Hypertrophy
Upright narrow QRS in V1
Left Ventricular Hypertrophy
Narrow QRS; Sum of V1/V2 + V5/V6 = 35mm or greater
Right Atrial Enlargement
Lead II: Tall p wave > 2.5mm
V1: Biphasic P wave w/ initial portion larger
Left Atrial Enlargement
Lead II: m shaped p wave > 0.12sec
V1: Biphasic P wave w/ larger terminal portion
