what is the mechanism for metabolic acidosis in ASA overdose?
Lactic acidosis.
Salicylates block oxidative phosphorylation in the mitochondria shifting cell metabolism to glycolysis. The end product of glycolysis is lactic acid.
Why do we give bicarbonate in ASA overdose?
To enhance secretion of salicylates from the tubules
ASA (weak acid) gets charged in a basic environment
charged molecules cannot pass be reabsorbed
why do confusion develop with ASA overdose?
decreased CNS glucose, cerebral edema and CNS toxicoty
indications for dialysis with ASA overdose:
seizures & coma, pulmonary edema, severe confusion
