12
Q

what is the mechanism for metabolic acidosis in ASA overdose?

A

Lactic acidosis.
Salicylates block oxidative phosphorylation in the mitochondria shifting cell metabolism to glycolysis. The end product of glycolysis is lactic acid.

13
Q

Why do we give bicarbonate in ASA overdose?

A

To enhance secretion of salicylates from the tubules
ASA (weak acid) gets charged in a basic environment
charged molecules cannot pass be reabsorbed

Q

what is the most common cause of death in fires?

A

carbon monoxide posioning

17
Q

initial orders for a fire victim (inhalational injury)?

A

100% O2, ABG, ECG, CarboxyHb level, Chem 7

18
Q

How do you die from CO poisoning?

A

myocardial infarction

19
Q

What is the mechanism of cardiac ischemia?

A

CO prevents the release of O2 to the tissues

20
Q

what is the strongest indication to use hyperbaric O2 in CO poisoning?

A

Presence of ST depression or elevation (MI)

21
Q

what is the most common presentation of aortic stenosis that a patient from CO poisoning has?

A

angina
