What is evidence-based practice?
The explicit, conscientious use of current evidence in making decisions about care of patients, integrating individual clinical expertise with best available external clinical evidence.
What are the 4 pillars
External evidence
Individual practitioner perspective
Well informed patient (parent)
Other drivers e.g. NHS or insurance companies, financial barriers, home vs clinic visits, areas of greater social disadvantage have fewer services, customs/practice of a clinic “we’ve always done it like that”.
What are barriers to EBP?

Management inertia/ poor leadership
Availability of evidence
Knowledge-Practice gap
What are the five stages of evidence based practice?
- Identify what you need to find out
- Search for appropriate evidence (form PICO question, use internet, library)
- Critically evaluate the evidence (use critical appraisal skills aims + methodology, participants, intervention, analysis/outcomes)
- Put the evidence into a plan of action
(change management, workshops/seminars, clinical guidelines) - Evaluate the effects of any action taken (outcome measures, user consultation, clinical and managerial supervision)
Using PICO
P- Population or problem e.g. preschool children with DLD
I- Intervention e.g. parent delivered
C- Comparison/control e.g. therapist delivered
O- Outcome e.g. increased spontaneous use of structures
Example of a PICO question
Does PA intervention (I) increase speech intelligibility (O) in 5-year olds with SSD (P) compared to those who don’t receive it (C)?
For qualitative studies, PICOT is used. What is PICOT?
