Exam 2 Flashcards
(20 cards)
What are the 5 Ps of labor?
- Passengers (fetus and placenta)
- Passageway (birth canal)
- Powers (contractions)
- Position of mother
- Psychologic response
What happens in the first stage of labor?
Onset of contractions to full dilation of the cervix
Full dilation to birth is described as what stage of labor?
Second stage
What does the third stage of labor include?
Birth of the fetus until delivery of the placenta
What stage of labor begins with the delivery of the placenta and lasts until the woman’s considered stable?
Fourth stage
usually 1 hr after delivery
What are the maternal physiologic adaptations to labor?
- cardiac output increases 12-13%
- Heart rate increases slightly
- BP increases during contractions
- WBC count increases
- Temp may be slightly elevated
- Proteinuria may occur
- Gastric motility and absorption of soli food decreases. N/V
- Blood glucose decreases
Is the valsava maneuver recommended for women pushing in labor?
NO!!! Can cause fetal hypoxia
How are contractions timed?
From the beginning of the contraction to the beginning of the next contraction
What are the signs of preceding labor?
- lightening
- return of urinary frequency
- backache
- strong Braxton hicks contractions
- weight loss
- Surge of energy
- bloody show
- Cervical ripening
- possible rupture of membranes
What are the characteristics of true labor?
- regular contractions
- frequent, longer intense contractions
- discomfort in back that radiates to abdomen
- contractions increase with activity
- cervical changes
What does the nitrazine test check for?
the pH of ruptured membranes
A positive nitrazine test shows what pH level?
Alkaline
This class of meds can decrease maternal HR, RR and BP. It can also decrease fetal oxygenation….
Opioids
What should be monitored before and after in mum and baby when mum on opioids?
Maternal VS
FHR and pattern
What acronym is used for remembering FHR, causes and managements ?
VEAL CHOP MINE
Describe VEAL CHOP MINE
Variable deceleration
Early deceleration
Aacceleration
Late deceleration
Cord compression
Head compression
Okay!
Placental insufficiency
Maternal repositioning
Identify labor progress
No interventions
Execute interventions
Tell me about precipitous labor…
- lasts less than 3 hrs from o set of contraction to time of birth
- can be caused by cocaine use, placental abruption etc
- can complicate maternal and fetal
What does APGAR stand for?
Appearance
Pulse
Grimace
Activity
Respiration
Explain the APGAR score ranges
0-3 severe distress
4-6 moderate difficulty
7-10 minimal or no difficulty
What is the baseline measurement of a newborn’s weight?
5.5-8.8 lbs
