(29 cards)
Which patients need sterile procedures/
- immunocompromised
- burns
When do you use wet to dry dressing?
50% slough or edema
How do you document dressing changes?
time, date, initial
What is selective debridement?
sharps
When do you use cross hatching?
santyl and stuff that needs to really get into the wound
When should you not use sharps debridement?
- dry, stable eschar
- low ABI
What is profore? Who should use it? What different kinds are there? What should you consider before using it?
- compression wrap
- venous insufficiency/would
- 2 layer and 4 layer
- ABI (2 layer=.6-.8; 4=.8-1), ankle circumference (>18 cm), pt tolerance
What’s the difference between the Unna boot and profore?
- Unna: class 2; health care worker has to put it on; dry skin, can leave it on longer if the pt can’t do their own changes
- Profore: class 3
What can you use for class 2 edema?
- ace bandage
- Unna boot
- 2 layer profore
What can laser do for wound care?
- enhances healing
- scar management-keloid scars
What does negative pressure tx do?
- gets exudate out
- protects the area
What is negative pressure indicated for?
- heavy exudate
- pressure ulcers
- chronic, open wounds
What are contraindications for negative pressure?
-osteomyelitisn
What are indications for hyperbaric O2?
- diabetes
- arterial insufficiency
- burns
- radiation burns
- osteomyelitis
- inducing revascularization through O2 in system
What can ultrasound do for wound healing?
- increase circulation
- help move through phases of healing
What are contraindications and precautions for US?
- contraindications: cancer, decreased sensation, cellulitis
- precautions:infection, sterile gel
- avoid going back to inflammation
What is estim used for in wound therapy?
- edema
- pain
- increasing blood flow
- facilitates debridement
- pressure ulcers
- chronic wounds
- can be done w/ infected wounds
What are contraindications and precautions for estim?
- contraindications: cancer, pregnancy
- precautions: decreased sensation, bleeding, silver dressings
What is negative polarity used for?
- stimulating granulation
- increasing blood flow
What is positive polarity used for?
after proliferation phase
What do you do if you’re using estim and the wound hasn’t changed in 1 week?
change polarity
Describe diabetic ulcers. How do you assess them? What do you do to tx them?
- callus, circular/regular borders, Charcot’s, forefoot/midfoot/hindfoot, plantar surface
- monofilament, pulses, ABI, measure wound, smell
- offloading, debride callus, educate on checking feet, good footwear
What do you do for a pressure ulcer?
- pulsed lavage, negative pressure
- foam, pomagram (active dressing)
- education: positioning, off-loading, HOB30, no prolonged sitting
- rojo cushioning
What does a venous insufficiency ulcer look like? How do you treat the?
- irregular borders, shallow, red
- foam, hydrocolloid, silver dressing, compression, elevation, decrease sodium intake, pulsed lavage, estim
