How do you decide how to select an exercise?
- Whats the impairment you’re trying to address? (strength, ROM, poor motor control) - PRIORITISE
- Pain levels
- Stage of healing
- Post op restrictions
- Patient’s goals
What are tests that you could use to assess ROM, strength, motor control?
ROM: AMT/PMT/mm length tests
Strength: Obs (wasting), MMT, Dynamo, Fxnal testing, RM testing
Motor Control: Movement patterns, specific tests
When do you use ROM exercises and what are your options?
Use ROM post injury, post op, after a condition that affects ROM (eg. adhesive capsulitis)
Options:
- simple (ER, IR, flexion, etc) or functional ROM ex
- active, passive, active assisted
When might the RC need strengthening?
- post op/immobilisation
- impingement
- instability
What are ways to strengthen the RC?
- start with inner range/isometrics
> allows for good scap control
> avoids impingement
> good activation of supraspin
When to prescribe ex for scapulae?
- post injury
- immob
- surgery
What are the main movements of the scapulae?
- Upward/downward rotation
- Internal/external rotation
- Posterior/anterior tilt
How to determine if scap work is needed?
- Obs (Static + dynamic)
- Scapular assistance test
- Scap hold test
How do you decide when to add functional ex?
- Symptoms allow
- Strength allows
- Control has improved
How can you progress a functional exercise?
Reps
Resistance
Range
Speed/power
Functional position
Kinetic chain?
Use multiple joints working together synergistically
Sport/work/ADL specific
What does stability training do?
- challenges balance
- requires synergistic activity of stabilizers
- progress by training stability in functional position (eg. in provocative position for the shoulder)
