Exodontia Flashcards
(22 cards)
what are the indications for extraction of teeth?
Caries Periodontal disease Abnormal wear resoprtion trauma supernumeray suppletmental prior to bisphosphonates and radiotherapy
what 10 problems may arise when extracting teeth?
extract wrong tooth loosening of adjacent teeth damage to developing tooth follicle damage to adjacent restorations resistance to extraction fracture of the root displacement of the root teeth inhaled or swallowed damage to tissues fractured instrument
what structures may become damaged?
bone
soft tissues
nerves
blood vessels
what damage to bone may occur?
fracture of the mandible
fracture of the tuberosity
burns
bony fragments
How can nerves be damaged?
Neuropraxia
Axonetesmisis
Nuerotemesis
how can you manage damage to blood vessels?
identify where bleeding is:
bone: bone wax
Soft tissue: ligature/diathermy
pressure and LA
where can teeth be displaced into?
Lower teeth Lingual pouch:
upper teeeth: antrum or pterygomaxilliary space
how do you manage displaced teeth into lingual pouch?
need GA
how do you manage displaced teeth into pterygomaxilliary space?
access via ascending ramus Henry 1972 (via antrum)
what complication may arise when retrieving a tooth displaced in to the pterygomaxilliary space?
risk of damage to the venous plexus
how can damage to the dental follicle arise?
luxating deciduous teeth
surgical exposure of supernumaries
when is the caldwell luc approach needed?
when a tooth or root has been displaced into the sinus
Which factors can predispose to jaw fracture>
Osteomyelies FIbrous dysplsia Multiple Myelome Osteoporposis cyst timout Pagets OI
What incison can you do for extracting 8’s?
Wards and modified wards incison
What is the difference to wards and modified wards?
Wards: this has is an incison between 7 and 8
Modified wards allows more anterior access
What must you be careful with when using wards?
Buccal branch
What should you always do before sectioning a tooth?
Always need to ensure to has been mobilised and then section from furcation point
Hei should you section a horizontally impacted tooth?
Vertically from furcation point
Section crown and then roots
How do you remove a Distally inclined tooth?
Removal distal bone first as this will allow tooth to tip into the space
Crown is used as initial elevation point
Ensure tooth is mobile
Remove the crown to Allow access to roots
How do you extract upper third molars?
Ensure there has been adequate bone removal and always support posterior.
How should you extarct upper third molars that are buccally placed ?
Using elevators eg Warwick James Coupland
What is the risk of 8’s which are in line of the arch?
There is alot of buccal bone and this fracture of tuberosity risk
