16
Q

Intrinsic Pathway

A

12 -> 11 -> 9 -> 8 ->
9, 8, calcium and PF3 form a complex
start of the common pathway

17
Q

Common Pathway

A

10 -> 5 ->
10, 5, PF3, and calcium form a complex
2 -> 1 and 13 -> clot

18
Q

Extrinsic Pathway

A

3 -> 7
3, 7, and calcium form a complex
common pathway

19
Q

Prothrombin Time (PT)

A

give information about the extrinsic pathway; only plasma is used; sodium citrate used; .2 tissue thromboplastin - > .1 plasma; 10-13 secs for clot to form; affected by Warfarin (Coumadin); reported in INR

20
Q

Coumadin

A

interferes with the carboxylation of Vit K dependent factors to prevent the forming of a clot; therapeutic when the activity level of factor 2 is less than 50%

21
Q

INR equation

A

(patient’s PT in seconds/mean PT in secs for healthy normal) ^ISI

22
Q

aPTT

A

.1ml reagent -> .1ml plasma -> .1 ml CaCl2 -> clot

gives information for the intrinsic pathway; monitors Heparin

23
Q

Inhibitors of Coagulation

A

Anitthrombin

Protein C

24
Q

Antithrombin

A

inhibits thrombin, factor 12, 11, 10, 9

25
Protein C
has cofactor protein S; inhibits factor 5 and 8; VIT K DEPENDENT
26
Heparin
extracted from bovine lungs or porcine intestines; increases the activity of antithrombin; can be reversed through protamine sulfate
27
LMWH
administered subcutaneous; catalyzes the neutralization of factor X; not measured with aPTT
28
Fibrinogen Structure
Center E domain with 2 side D domains; contains Alpha, Beta , Gamma chains
29
Formation of a clot
- Thrombin takes fibrinogen and removes the a and b chains -- becomes fibrin - binds to another fibrin molecule through H bonds - Factor 13 is activated to cause covalent bond linkage between the D-domains - fibrin clot is formed
30
Fibrinolysis process
Plasminogen is changed to plasmin by T-Pa; plasmin is bound by antiplasmin
31
vWF structure
contains 1b/9 site for platelet binding; has collagen binding site for the site of cut
32
Thrombin Time
time for thrombin to convert fibrinogen to fibrin; reflects the fibrinogen concentration
