What does the basic Fetal Anatomy scan systematically check for?
Brain/Face and Lips, heart, chest, umbilical insertion, abdomen, kidneys, bladder, limbs, spine, placental site
What do we want the NT to be less than? And what can this show?
3.6mm, cardiac problems or Trisomy
Who can’t we offer the trisomy blood test to?
Mum has Down syndrome, other maternal problems that might skew the results.
What is the percentage abnormalities can be picked up my Fetal anatomy scan?
50%
Who manages Fetal growth restriction?
Management with MDT including Fetal medicine Team/ Obstetric Team/ Neonatologist
Risk factors for Fetal growth restriction
Age, Smoker, Previous FGR, Malnutrition, Alcohol, PET, Hypertension, SGA (Saving babies lives: Version 3)
How do we identify FGR
Weight below the 3rd centile, Abnormal uterine Doppler (shows the blood flow to baby from maternal uterine),
Abnormal umbilical Doppler
How much do you scan someone with FGR
Weekly for FGR for blood flow, growth scans 14 days apart
What scans generate a weight?
Head circumference, abdominal circumference, femur circumference
What gestation would you deliver a small baby (below 3rd) with abnormal blood flow
36 weeks
Why can oligohydramunous and polyhydraminos happen?
Infection, structural (kidneys not working, not swallowing etc), chromosomal, placental (FGR), others- ?SROM/?GDM, ?multiples
No movement and polyhydraminos could be neurological issues with baby as they are not swallowing
What are the concerns with multiple pregnancies?
Liquor discordance
TTTs/TAPs/sFGR
IUGR
APH
PET
VTE
Preterm Labour
