Types of Initial Discovery Instructions

A
  1. the name, address, and telephone number of individuals likely to have discoverable information to support claims or defenses, excluding those used solely for impeachment.
  2. A copy or description of all documents, electronically stored information, and tangible items in the disclosing party’s possession that may support its claims or defenses, except if used solely for impeachment.
  3. A computation for each claimed damage category and supporting documents, unless privileged, must be provided. This includes evidence related to the nature and extent of injuries. However, no computations for noneconomic damages are required; the party must only identify claimed damage categories and supply supporting documents.
  4. A copy of any insurance policy that may cover all or part of a possible judgment or reimburse payments made to satisfy the judgment.
