65yo man with poorly controlled DM presents with painful swallowing (Odynophagia) and white plaques (likely candidiasis) seen on endoscopy
Oesophageal Candidiasis
40yo man with untreated HIV presents with odynophagia. Endoscopy shows large linear ulcers; biopsy confirms CMV
CMV Oesophagitis
47yo kidney transplant recipient on immunosuppressants developed painful oral ulcers followed by odynophagia and retrosternal discomfort. Examination revealed gingival vesicular lesions, and endoscopy showed oesophageal ulcers
Herpes Gingivostomatis and Esophagitis
25yo man presents with periumbilical pain that shifts to right lower quadrant, along with Nausea and low grade fever. PE: Tenderness at McBurney’s point and mild guarding
Appendicitis
55yo man with known liver cirrhosis presents with increasing abdominal distension, fever and confusion. On examination, no guarding or rebound tenderness, ascitic tap shows cloudy fluid with neutrophils >250/mm3
Spontaneous/ Primary peritonitis
60yo woman presents with sudden severe abdominal pain, fever and rigid abdomen with guarding. CT scan shows perforated sigmoid diverticulitis with free air in peritoneal cavity
Secondary Peritonitis
Qn: 70yo presents with fever, RUQ pain and malaise. US abdo reveals well-defined fluid-filled lesion in the liver
